About Us
The Healthcare Consortium is a rural health network made up of your friends and neighbors from the health care, business and consumer sectors. It is one of 34 such networks throughout New York State and receives funding from the New York State Department of Health, Office of Rural Health, and through public and private resources and donations.

Our Services
The Healthcare Consortium has been improving access to health care for Columbia County residents with programs and initiatives that meet identified community needs.

The Medical Transportation Service provides transportation to and from health appointments for residents with no other means to get there.

Our Community Partnership for Children's Dental Health provides school-based screenings and limited dental care and parental education about practicing good oral hygiene.

Our health education Speakers Bureau offers professional presentations by experts on a wide variety of health topics.

Our new Childhood Obesity Prevention program will strive to promote healthier childhood lifestyles.

Now we are also serving the residents of Greene County with many of our programs that increase their access to health care.

Covering the Uninsured, the Columbia-Greene Facilitated Enrollment Program, is helping eligible individuals and families get affordable health insurance plans such as Child Health Plus, Family Health Plus, and Medicaid.

Our Healthy People Partnership combines two excellent programs for FREE breast, cervical and colorectal cancer screenings. Prostate screening information is also available.

Our Cancer Support Program for Columbia and Greene Counties provides information and referrals to services available in the Capital Region and a peer mentoring network to provide one-to-one support for individuals affected by a cancer diagnosis. Support groups for adults and children are major components of this program.

The Columbia-Greene Diabetes Coalition offers monthly education and support meetings which alternate between the counties. In odd months
(Jan, Mar, Mat, etc) the meetings are held in the Human Services Building in Hudson, and in even months (Feb, Apr, Jun, etc) the meetings are
held in the Greene County Office Building on Catskill.

The Rip Van Winkle Tobacco-Free Coalition provides education & advocacy to reduce the social acceptability of tobacco use, and promotes cessation resources.
