The Healthcare Consortium is a rural health network made up of your friends and neighbors from the health care, business and consumer sectors. It is one of 34 such networks throughout New York State and receives funding from the New York State Department of Health, Office of Rural Health, and through public and private resources and donations.

COLUMBIA COUNTY
The Healthcare Consortium has been improving access to health care for Columbia County residents with programs and initiatives that meet identified community needs.
The Medical Transportation Service provides transportation to and from health appointments for residents with no other means to get there.
Our new Kids in Motion program takes on childhood obesity prevention with the goal of will promoting healthier childhood lifestyles.
The NY Connects, Columbia County information resource providing long term care services information and assistance for individuals and families in Columbia County.
Our health education Speakers Bureau offers professional presentations by experts on a wide variety of health topics.
GREENE COUNTY
Now we are also serving the residents of Greene County with many of our programs that increase their access to health care.
Covering the Uninsured, the Columbia-Greene Facilitated Enrollment Program, is helping eligible individuals and families get affordable health insurance plans such as Child Health Plus, Family Health Plus, and Medicaid.
Our Cancer Services Program of Columbia and Greene Counties combines two excellent programs for FREE breast, cervical and colorectal cancer screenings. Prostate screening information is also available.
The Rip Van Winkle Tobacco-Free Action provides education & advocacy to reduce the social acceptability of tobacco use, and promotes cessation resources.
