Kid in Motion Mini-Grant Application
Funding is available to Columbia County School Districts for programs that promote healthy eating and physical activity, for kids in grades K-8.
Three school districts will be awarded up to $5000 each for the 2010-2011 school year, for before and after-school physical activity programs. Application deadlines for schools are October 15, 2010 and January 15th, 2011.
School districts can apply throughout the school year for other health promotion initiatives. Applications will be considered on a first come, first serve basis and funding will be awarded as available. Successful applications will include information on organizational policies that ensure healthy food offerings during program activities and/or in vending machines. Awardeeswill be required to submit quarterly reports and vouchers for reimbursement of approved program expenses.

Please submit a narrative proposal using the following format:
1. Does your district currently offer before or after school physical activity programs for children in grades K-5? (Please include duration, type of activity, # of weeks, # children participating and grade levels.) Pleas indicate the activities that have been funded, in part or fully, by a Kids in Motion grant or any other grant.
2. Is the proposed program new or is it enhancing an established program?
3. How manystudents will be served by the proposed program?
4. Which grades?
5. What will be the duration of the program? (Please include days/week, hours per session, # of weeks)
6. Please give a detaileddescription of the program.
7. Provide district wellness policies that specify guidelines for healthy snacks and/or vending or describe a plan to ensure that food and drink offerings are nutritious.
8. Kids in Motion grant funds are intended to support the establishment of physical activity clubs and programs, Applicants need to develop a plan for sustaining the program(s) when funding is no longer available. Describe your sustainability plan for the activities you are proposing.
9. Who will oversee implementation of the program, communicate with the Program Coordinator and meet quarterly reporting requirements? (Please include phone # and e-mail address)
10. Applicants are required to contribute matching funds or in-kind support equal to 50% of the requested dollar amount. (value of volunteers time, staff time, equipment purchase, other funding sources, etc)

School-based Mini-grant guidelines
GOAL: K-5 Students will have the opportunity to engage in 30 minutes of non-competitive physical activity/physically active free play before or after school or during summer school program three times per week.
- Funding is intended to provide the financial support needed to create before or after school non-competitive physical activity programs.
- Funding is intended to cover the costs of staffing, supervising and coordinating the program, with “other” costs kept to a minimum.
- Programs will be individualized to fit in with transportation, space and other site-specific factors.
- Programs that will introduce students to games and sports they may not be familiar with and that will promote an environment of “having fun while being physically fit” are encouraged.
- Community volunteers may be recruited to offer special activities such as: yoga, kickboxing, hip-hop dance, etc.
- High school student helpers may be hired to assist with supervision and help with skill development.
Example Program:
- 3 days/ week (M, W, Th) for 30 minutes
- 36 weeks total: 4-week rotations (9)
- Activities to include a combination of the following: M-Game Day; W-Mileage Club; Th-Swimming
- Grades 2-5; “x” # students

*Consideration of the following will be taken into account in awarding funds: Number of students to be served, Staffing, and Pre-existing programs.
*The over-all goal is to provide opportunities to the maximum number of students.
