Dear Friends,

This directory, developed by the Healthcare Consortium and funded by the New York State Department of Health, has been designed as a resource tool to help educate and empower cancer patients, their families, friends, and caregivers to become active participants in their medical care.

The mission of the Columbia County Community Health Care Consortium, Inc. is to increase access to quality healthcare through collaboration, education, information, and service delivery. We believe that individuals are equal partners in their own health care and require information and education to fulfill their role effectively.
