Caregiver Support Program:
Connections for Caregivers

The Connections for Caregivers program is intended to provide information to caregivers about available services, assistance in gaining access to the services, individual counseling, caregiver training to assist the caregivers in making decisions and solving problems relating to their caregiving roles, organization of support groups and respite.

For family caregivers, the program will help sustain their efforts to care for an older relative who has a chronic illness or disability and will promote the ability of these individuals to remain in their homes and local communities, instead of being placed in residential facilities.

Priority is given to older individuals age 60 and over with greatest social and economic need, to rural, socially isolated and minority caregivers and to individuals providing care to persons age 60 and older, with mental retardation and related developmental disabilities.

The Respite and supplemental services provided through this program may only be provided to a frail older person. The care-receiver must be unable to perform at least 2 activities of daily living without substantial human assistance, including verbal reminding, physical cueing or supervision, or the older person has a cognitive or other mental impairment that requires substantial supervision because the person behaves in a manner that poses a serious health or safety hazard to the person or to another person. An in-home assessment is required.

When appropriate, limited Psychotherapy services will be provided but will be capped at $500.00 per caregiver, per year.

Respite Care will be restricted to In-Home respite services with a maximum of 72 hours per year, per caregiver. If funding allows, the 72-hour limit will be expanded. There is a suggested contribution to caregivers of $4.00 per hour for respite.

Funding for Assistive Devices will be provided to those individuals needing this service. Funding is limited to $50.00 per person, per year.

Funding for minor home modifications to make the home handicapped accessible or for health and safety reasons is limited to $100.00 per person, per year to those individuals needing this assistance.

The Respite Program is designed to give the caregiver at home a rest. A warm, welcome awaits the Respite Guest for periods of 1-30 days. Includes: 24 Hour Nursing Care, Rehabilitation Services, Activity Programs, Social Services, Therapeutic Dietary Services, and all necessary Laundry and Housekeeping Services. Suites include a private bathroom, telephone, and television.

Eden Park is a certified Scheduled Short Term Care (Respite) provider. Respite costs can be covered by Medicaid or through private resources.
