NY Connects, Columbia County provides referrals to agencies that may help in the care of your loved one. The NY Connects staff can assist with these referrals, avoiding duplication of efforts and repetitive information.

NY Connects, Columbia County provides assistance in obtaining services. The program does not provide direct services or financial support beyond an initial general assessment of needs to determine appropriate referrals.
